A health history and Nutrition intake form are required for all meal plan purchases and initial nutrition consultations. To help us ensure the highest level of security and compliance, in protecting your child/adolescent’s/your health records we use HIPPA compliant forms and abide by this information is for the sole purpose of providing the proper age-appropriate nutrition education material, meal plan or health product in order to personalize your experience (collectively the “Health Information.”)
